Hazard risk assessment - Changing a hazard criteria rating and clicking Save icon

‘risk assessed by’ will show the user who amended the assessment with the assessed date

‘Last updated’ will display the user who last saved the record which would be same as ‘risk assessed by’

Updating hazard details after a risk assessment

‘Risk assessed by’ will display the user details of the last user who changed details in the risk assessment.

‘Last updated’ will display the user who last saved the record along  with the date.

Submitting the Hazard assessment for signoff

‘pending signoff’ will display the name of the signoff authority

‘Last updated’ will display the user who last saved the record along  with the date

Updating hazard details during ‘pending signoff’ stage

‘pending signoff’ will display the name of the signoff authority

‘Last updated’ will display the user who last saved the record along  with the date

Hazard risk assessment during ‘pending signoff’

‘pending signoff’ will display the name of the signoff authority

‘Last updated’ will display the user who last saved the record along  with the date (last assessed)

When Hazard signoff is approved

‘approved by’ will display the Signoff authority name and the signed off date.

‘Last updated’ will display the user who last saved the record along  with the date (in this case same as ‘approved by’)

When Hazard signoff is declined

‘declined by’ will display the Signoff authority name and the signed off date.

‘Last updated’ will display the user who last saved the record along  with the date (in this case same as ‘declined by’)

Updating hazard details after signoff

Signoff authority name and the signed off date will be displayed in ‘approved/declined by’ line.

‘Last updated’ will display the user who last saved the record along  with the date
